All players on National (Orange and Navy Teams) 13's - 17's need to fill out the annual JVA online Medical release forms.

You have to create a JVA registration for both yourself and your child. If you have done this in the past and remember your registration information.. you can just log in to JVA for this...

You are free to update what information is displayed by maintaining your daughters information when you log in to the Team Z website. Forgot your user name or password? Just send us an email, we can provide you what you need!
